The Information We Collect

This notice applies to all information collected or submitted on our website.  It does not apply to records we create and information we collect in sessions with our clients, such as treatment notes.  If you have any questions about your client file and related policies, please contact us.

On some of our website pages, you can provide contact and personal information related to you and your family members.  For instance, our contact form collects your name, email, and phone number.

We only have access to—and collect—information that you voluntarily give us via our online forms, email, or other direct contact from you. We will never sell this information to anyone. We use the information in two ways:

  1. When you submit an informational form, we use the information provided to follow-up with you. By completing the form, you are giving us permission to contact you with more information.

  1. When you complete a form relating to treatment history, treatment concerns, and goals, we will use this information during your evaluation and future treatment.

We will not share your information with any third party outside of our organization, other than as necessary to fulfill your request, e.g., if you provide a release permitting us to collaborate with your child’s school.

Unless you ask us not to, we may contact you via email in the future to tell you about news related to our business, specials, new services, or changes to our policies.

Your Access to and Control Over Information

You may opt out of any future contacts from us at any time.  You can also contact us (see contact information, below) at any time to see what information we have about you, if any; to change/correct information we have about you; or to ask us to delete information we have about you.
